Once You Book Your Initial Assessment

You’ll receive an email confirmation with a link to complete the initial forms. It is imperative you complete these forms at least 3 days prior to your visit. It will take approximately 45 minutes to complete, but this information will help me get to the root of your concerns.

Your Initial Assessment

Health is a journey and the initial plan is just the beginning. Throughout our work together, you will be tracking various symptoms and progress. We will also discuss various lab testing, where needed, to better diagnose the root cause as well as track internal changes.

You can’t change something you can’t see. Tracking is essential to get a true understanding of how your health is changing and responding to treatments.

Patient resources in the Patient Portal will be made available to coach you between visits.

Initial Treatment Plan In Your Portal

You can find your detailed and strategic initial treatment plan in the secure online medical portal called Jane App. Links to videos, podcasts and traking tools will also be there to increase your success through this health journey.

My Expectations of You

○  You take an active role in your health.
○  Tracking symptoms, noting your menstrual cycle, listening to what foods are helpful (or harmful) to you.
○  You make space and time for healing.
○  Just meeting with me is not going to solve all your concerns and optimize your performance.
○  In between sessions you must schedule time to work on your treatment plan and goals. This is where the magic happens!

Your Expectations of Me

○  I’m going to listen, ask questions and seek clarity on your health and goals.
○  I’m here to support you in the health journey in ways I know how. If I don’t then I’ll refer or share resources of how/who can better assist you.
○  I will ensure proper communication of appointments, billings, lab information, treatment plans in a clear and timely fashion.

I am NOT the best fit for you IF….

  • You are not willing to change your current habits – food, sleep, relaxation, self-care, etc

  • You are not willing to keep an open mind

  • You are unwilling to add physical movement or activity to your life

What you need to do BEFORE the INITIAL visit

• Complete the forms

• Sign the release of records

• Take photos of pharmaceutical and supplements (front and back of label)

• Start to track where you are in your menstrual cycle (if applicable). You can use pen and paper or any app you like. My favourites are FITR woman, FLO, and Wild AI

What you need to do before ALL visits

• Take photos of pharmaceutical and supplements (front and back of label) if any have CHANGED since we last met

• Bring in your tracking forms (these will be discussed during your visit so you know what and how to track)

• Know where you are in your menstrual cycle (if applicable). If you have ovaries, you have a monthly cycle.

How do VIRTUAL visits work?

• 1:1 visits happen through the Jane apps secure portal.

• You will receive an email from Jane app with the link to your appointment. Click the link, sign into the secure portal.

• You may need to download the app onto your device so log in a few minutes early for your first visit.

• Have your camera on.

• Once logged in, you will directly to a virtual waiting room with the notice “waiting for practitioner”. Don’t worry I’ll be there shortly. I try to run on-time but I’m not always perfect, so hang tight.

• This secure portal is great because you can book your appointment, we can meet, share your treatment plan, lab results and other ducuments AND you can pay your invoice online.

What is the FEE schedule for services?

• The initial visit dives deep into your health history, lifestyle and exercise habits as well as your health goals. This visit is up to 90-minutes in length. Fee is $349

• Regular follow-ups are up to 30-minutes. We can review blood work and make adjustments to your treatment plan. Fee is $115

• Momentum visits are brief check-ins to ensure you’re on track. These visits are up to 15-minutes. Fee is $60

• Extended follow-ups are up to 45-minutes to address a new health concern. Fee is $170.

Payment info

• Payment for appointments is required at the end of each visit by Visa or Mastercard.

• While OHIP does not cover naturopathic services, many private insurance policies offer partial or complete coverage. Please check your policy to see whether you are covered for naturopathic medicine.

• Official receipts will be issued at the end of each visit so that you may be reimbursed directly by your insurance company.

• Any supplements or remedies suggested can usually be purchased at the clinic dispensary or through the online dispensary. Every effort has been made to ensure that all products are of the highest quality and of reasonable cost. You are, of course, welcome to purchase your supplements elsewhere.

• If you are unable to make a scheduled appointment, please let us know as soon as possible so that we can provide care to another patient. Please provide at least 2 business days' notice or full visit fees will apply.

How do I pay for a visit to a naturopathic doctor?

• Visits to an ND are not covered by OHIP. However, most extended health insurance plans do cover the cost of seeing an ND.
